At least five people have been killed after an Amazon Prime Air Boeing 767 overran a runway at Miami International Airport, crossed into a ground-traffic area, struck vehicles and caught fire. Five others were taken to hospital, three in critical condition. The scale of the disaster is confirmed; its cause is not.
Prime Air Flight 7598 landed shortly before 2 p.m. EDT on September 6 after flying from San Juan, Puerto Rico. The 767-300 freighter was operated by cargo carrier 21 Air on Amazon's behalf. Miami-Dade officials said the aircraft left the usable runway and hit multiple vehicles before coming to rest with fire and heavy smoke.
More than 60 emergency units and roughly 200 personnel responded. Firefighters dealt with trapped occupants and an active fuel leak. The airport initially stopped all flights before reopening two of its four runways later Sunday. The disruption produced extensive cancellations and delays.
Officials did not immediately identify the dead or say definitively whether each person was aboard the aircraft or in vehicles struck on the ground. Early reports indicated the pilot and co-pilot were trapped in the plane and at least two people were trapped in vehicles. Those details may change as hospitals, investigators and families complete identification.
Flight-tracking data cited by Reuters showed the aircraft moving at about 112 knots—129 miles per hour—when it exited the usable runway. Speed at that single point does not explain why it happened. Investigators need the touchdown location, braking configuration, reverse-thrust performance, tire condition, runway surface data, wind and cockpit recordings.
Thunderstorms and gusty weather were present in the region, leading to immediate speculation about hydroplaning or unstable approach conditions. Weather may prove relevant, but aviation investigations resist monocausal explanations. A runway overrun can result from a long or fast landing, delayed braking, systems failure, contaminated pavement, wind shift, decision-making or several factors interacting within seconds.
The aircraft was 32 years old and had flown as a passenger jet before conversion to cargo service in 2015. Age alone does not make an aircraft unsafe. Commercial jets operate under inspection and maintenance programs based on cycles, hours and structural condition. Investigators will examine records rather than infer failure from the manufacturing year.
The National Transportation Safety Board sent a team led by Chair Jennifer Homendy and planned an initial briefing. Its work will include recovering the cockpit voice and flight-data recorders, documenting wreckage, interviewing surviving crew and witnesses, and testing relevant components. A preliminary report can establish a factual sequence, while a probable-cause finding may take many months.
Amazon does not operate every aircraft carrying its brand. Contractors such as 21 Air fly within the logistics network, making oversight a central question. Investigators will consider operator procedures, training and maintenance, while regulators may review whether commercial pressure or scheduling played any role. There is currently no evidence establishing that it did.
The crash will revive attention to the 2019 loss of Atlas Air Flight 3591, another Boeing 767 operating for Amazon, which killed all three aboard in Texas. Shared aircraft type and customer do not prove a common cause. The earlier NTSB investigation identified a specific loss-of-control sequence, not a generic flaw that automatically explains Miami.
Runway design will receive scrutiny because the aircraft continued into areas used by road vehicles. Investigators may assess runway-end safety provisions, drainage, pavement condition and whether physical geography limited an arresting area. That analysis should not imply the airport caused the crash; safety investigations examine barriers that could reduce harm even after an approach or aircraft problem occurs.
Cargo aviation often operates at night, under tight schedules and with older converted aircraft, but the sector remains subject to federal rules. The relevant question is whether this flight's risk controls worked—not whether freight operations as a whole are inherently unsafe.
Investigators will also study evacuation and rescue timing. Ground vehicles were exposed beyond the runway, and an active fuel leak complicated access. Identifying which deaths resulted from impact, fire or delayed rescue can lead to different recommendations for airport layout, emergency staging and vehicle controls.

The most important question is not whether a dramatic video appears to show bad weather or excessive speed.
